Support coordination is the part of a plan that pays for someone to help you use the rest of it. A support coordinator finds providers, sets up service agreements, keeps the budgets on track, prepares you for plan reviews and steps in when something breaks down. ResicareSA provides Level 2 support coordination across Adelaide and regional South Australia, delivered by coordinators who live and work here.
You do not have to use any other ResicareSA service to have us coordinate your plan, and we will tell you when another provider is the better fit for a support. That independence is written into how we work, because a coordinator who only ever recommends their own employer is not coordinating anything.
What a support coordinator does
The work is practical. In the first month it is mostly setting things up; after that it is keeping them running and preparing for the next plan.
- Explain the plan: what each budget is for and what it can and cannot pay for
- Find and compare providers, including for housing, therapy and community supports
- Set up service agreements and schedules of support
- Track spending against the plan so money does not run out early or sit unused
- Coordinate between providers, hospitals, schools and the NDIA
- Prepare evidence and reports for plan reassessments
- Respond when a support breaks down, including crisis situations
Home and living requests
Because ResicareSA runs SIL and SDA homes, our coordinators know the home and living process from both sides: what the NDIA asks for, what a usable roster of care looks like, and which SDA categories exist in which parts of the state. That is useful whether or not one of our homes is the answer. If it is not, we say so, and help you find one that is.
Across South Australia
Coordination is delivered by phone, video and in person. Our coordinators cover metropolitan Adelaide, the Adelaide Hills, the Yorke Peninsula and Mid North, Whyalla and Port Augusta, the Riverland and the South East. Knowing which providers actually have capacity in Port Augusta or Berri is most of the job in the regions, and it is knowledge that cannot be looked up.
How to start
Check your plan for a Support Coordination line under Capacity Building. If it is there, call or enquire and we will arrange a first meeting within a week. If it is not, we can explain how to ask for it at your next plan meeting or through a change of circumstances.
Common questions
What is the difference between a support coordinator and a plan manager?
A support coordinator helps you find, set up and manage your supports. A plan manager pays the invoices and tracks the budgets. They are separate lines in a plan, and you can have either, both or neither. ResicareSA provides both, separately.
Can I use ResicareSA for support coordination and other providers for everything else?
Yes. Most of the people we coordinate for receive their other supports elsewhere. Our coordinators recommend whichever provider fits, including competitors.
Do you offer specialist support coordination?
We provide Level 2 support coordination. For Level 3, specialist support coordination, we will refer you to a provider who holds it, and we work alongside specialist coordinators regularly.
Do you cover regional South Australia?
Yes: Whyalla, Port Augusta, Port Pirie, the Copper Coast, the Riverland, Mount Barker and the South East, by phone, video and in person.
