If you wish to become an NDIS participant, you must submit an access request by either:
Contacting the NDIA at 1800 800 110 and requesting to make an access request
Completing and submitting the Access Request Form and sending it via email to NAT@ndis.gov.au
Confirm your identity and/or a person's authority to act on your behalf.
Answer questions about the NDIS access requirements, including age, residence, and disability.
Provide consent to access the NDIS and seek information from third parties.
You may also need to provide additional information about your disability and how it impacts your daily life, supported by copies of existing documents such as letters, reports, or forms signed by your current healthcare practitioner.
