ResicareSA holds the Assist-Life Stage, Transition registration, and in practice this is the support people ask for when something is about to change and the existing plan will not survive it. We work across Adelaide and country South Australia, and because we also run housing and deliver daily supports, a move that touches all three does not have to be coordinated across three organisations.
Transitions are where plans most often come apart. Leaving school, moving out of the family home, coming out of hospital, changing providers, a parent's health changing, or moving from a child's plan to an adult one — each of these changes what support you need, and usually all at once.
This support is the planning and coordination around that change. We work with you and the people around you to map what is coming, what needs to be in place before it happens, who is responsible for each piece, and what the fallback is if something slips. That includes talking to schools, hospitals, other providers, family and your support coordinator so the handover is planned rather than improvised.
The earlier this starts, the better it goes. A move out of home is easier to get right with six months of preparation than six weeks.
